Research Project is a Senior High School applied subject that

aims its students to be familiar with and be exposed to the
rigors of Qualitative and/or Quantitative Research.

In this regard, we, the 12-STEM researchers of the study entitled

“EFFECTS OF SPIRAL CURRICULUM APPROACH IN MATHEMATICS AMONG
GRADE 10 STUDENTS IN GABALDON VOCATIONAL AGRICULTURE HIGH
SCHOOL”, in partial fulfillment of the requirements in our
subject Research Project, are asking for your permission to let
us distribute survey questionnaires containing thirty (30) items
to your Grade 10 students to make it possible for them to become
respondents of the study.

Furthermore, we are humbly informing you that the Grade 10

respondents will be required to sign a consent letter that we,
the researchers, will include together with the printed survey
questionnaire. This is for the reason that we need their
permission to have a copy of their SF9 or Form 138 and SF10 or
Form 137 in order to gather their first quarter Mathematics
grades from Grades 7, 8, and 9, as well as their current first
quarter Mathematics grade in Grade 10. Due to the necessity of
knowing the respondents' identities for the study, the names,
grades, and sections of the respondents included in the first
page of the printed copy of the survey questionnaire form must be
filled out.

Rest assured that the information to be provided will be treated

with complete confidentiality and will only be used for the sole
purpose of the study.
