AFFIDAVIT OF UNDERTAKING

I, BRENDA G. JUMAWAN, Filipino, of legal age, married and a resident of Cogon, Inabanga, Bohol,
after having been sworn in accordance with law, hereby depose and say:

1. That I am the Attorney-In-Fact of my husband, Jonathan B. Jumawan, empowered among others to

mortgage our KAWASAKI KMX 125 motorcycle bearing Plate No. A70800 duly registered with the Land
Transportation Office (LTO), Tagbilaran City;

2. That I hereby voluntarily UNDERTAKE that upon release of the Certificate of Registration of our
above-mentioned motorcycle by LTO Tagbilaran City, I will submit the same to SOCIAL DEVELOPMENT
LENDING, INC., Tagbilaran City, in compliance to its loan requirements;

3. That I execute this affidavit voluntarily to attes to the truth of the foregoing statements and for
whatever legal purpose this may serve.
