AFFIDAVIT OF UNDERTAKING

WE, ARNOLD GARRIDO, JASON ABANTO and JHAM OLIVER ARBAS, Filipinos, 15
years old, all single, and residents of Totolan, Dauis, Bohol, after being sworn in accordance
with law, hereby depose and say:

That we are all 2nd year High School students of Tabalong High School, Tabalong,
Dauis, Bohol;

That we promise to be a good student and show interest and be diligent in our studies;

That we will obediently follow and abide all school rules and regulations;

That if ever we cannot comply with the foregoing conditions, we undertake to willingly
transfer to another school;

That if ever we cannot comply with the foregoing conditions, we undertake to willingly
transfer to another school;

That we execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
