AFFIDAVIT OF UNDERTAKING

I, SOFRONIO BAUTISTA JR., Filipino, 14 years old, single, and a resident of Booy
District, Tagbilaran City, Bohol, after having been duly sworn to in accordance with law, do
hereby depose and say:

That I am a 2nd year High School student of San Jose National High School, Talibon,
Bohol and intends to transfer to Holy Name University (HNU) Night High School, Tagbilaran
City, Bohol;

That I promise to be a good student and show interest and be diligent in my studies;

That I will obediently follow and abide all school rules and regulations;

That if ever I cannot comply with the foregoing conditions, I undertake to willingly
transfer to another school;

That I execute this affidavit voluntarily to attest to the truth of the foregoing statements
for whatever legal purpose this may serve.
