Re: Mrs Pamela Viduka

I am writing to update you about Mrs Pamela Viduka, who is scheduled to be


discharged on 24th November, 2008 and requires continued care and support from
your facility following recent mitral valve replacement surgery.

Mrs Viduka was admitted with complaints of fainting, light-headedness, along


with chest pain. She was diagnosed with mitral valve prolapsed, for which she
underwent mitral valve replacement surgery on the same day.

During hospitalization, she has made routine recovery followed by antibiotics,


analgesics, and anti coagulant therapy. She was also assisted with her personal care
such as showering and dressing. She was also reviewed by dietician who advised
her to take low salt diet which may be due to her elevated blood pressure. As you
probably know she is hypertensive.

Since being admitted for a week, Mrs Viduka has recovered well and can walk
using stick or frame. She has been advised to take rest and avoid invasive surgical
or diagnostic procedures until prophylactic antibiotics are administered. Please
monitor her compliance with medication. She is prescribed warfarin; salpetrol, 3
puffs and mirax, 25mg, to be taken daily. It would be appreciated if you could
provide assistance as per her need and also ensure that INR test and follow up
examinations are conducted regularly.
