DISCHARGE PLAN:

MEDICATION:
● Explain the importance of regularly taking prescribed medications.
● Advise the client not to miss the intake of medications given by her physician upon
discharge.
● Discuss the possible reactions and side effects of medications that might produce. Seek
medical attention if this arises.
● Instruct the client to continue with follow up medical care.

ENVIRONMENT:
● Maintain a quiet and pleasant environment to promote relaxation.
● Advise or schedule periods of uninterrupted sleep or rest.
● Identify the resources to help with physical therapy, and the support of family and friends.

TREATMENT:
● Discuss the importance of strict adherence to medication regimen to ensure complete
healing.
● Discuss the purpose of treatments to be done and continued and report to the health
provider any complication that may arise.
● Instruct the client to understand and follow discharge instructions religiously and accurately.

HEALTH TEACHING
● Observe for general health measures (adequate sleep, proper diet, and maintaining a safe
and clean environment)
● Discuss to the client the importance of good hygiene like handwashing, oral care, perineal
care and other hygiene measures.
● Advise the client to refrain from straining activities and encourage ambulation as a form of
light exercise.
● Advise the client not to engage in sexual contact while being treated.
● Provide written and oral instructions about medications and follow up visits.

OUTPATIENT REFERRALS:
● Advise client to go back to the hospital on a specific date to have a follow up check up after
discharge.
● Refer client to social services and support groups that could assist them financially and
psychologically.
● Consult a doctor for any problems or complications encountered.

DIET:
● Identify foods rich in protein, potassium, vitamins and minerals for the client.
● Encourage client to drink plenty of water and limit the alcohol and caffeine consumption.
● Advise client not to skip meals.
● Advise client to maintain a healthy and balanced diet.
● Advise client to avoid processed foods and fast food.

SPIRITUALITY/SEXUALITY
● Refer client to a spiritual counselling.
● Advise client to pray and stay connected to God.
● Advise the client’s significant others to accompany and support them in any way they can.
● Advise client and significant others to maintain a good and peaceful environment.
