I, the undersigned, hereby acknowledge the receipt of the above-listed company access property. I understand that
this asset is the property of Digiphoto Entertainment Imaging Sdn Bhd and has been issued to me for the sole purpose
of performing my job duties.

I agree to the following terms and conditions:

1. I will use the company asset exclusively for work-related purposes in accordance with company policies.
2. I will take all necessary precautions to safeguard the asset from damage, loss, or theft.
3. I will report any damage, loss, or theft of the asset to my supervisor or the appropriate department
immediately.
4. In the event the asset needs repair or replacement, I will be required to return the asset in exchange for a
replacement.
5. I understand that I am responsible for returning the asset in the same condition it was issued to me upon
termination of my employment or when requested by the company.
6. I acknowledge that any unauthorized personal use or misuse of the asset may result in disciplinary action,
including the possibility of termination.
7. In the event of the loss, irreparable damage of the asset due to my negligence or violation of company policies
and failure to return on termination of employment or anytime on demand I agree to pay a fine as below:
