The above-mentioned employee is authorized to personally use the company vehicle from

1. Obey traffic laws in your jurisdiction and be courteous toward other drivers.
2. Employee shall pay for the use of fuel and toll fee and minor repairs.
3. Employee will be held accountable for any damages or cleaning fees incurred while using
the vehicle
4. Employee agrees to return the vehicle in its current condition

I have read, understand, and agree to comply with the above conditions, in exchange for being
