    NOTICE OF UNDERTAKING

    Midyear, SY 2022-2023

    I, John Enrick G. Manuel, a/an Entrance Scholarship Program grantee, am requesting the
    approval of the down payment Promissory Note for the Midyear Term, SY 2022-2023 pending
    the release of the final grades of the Second Semester, SY 2022-2023.

    In the event that I fail to meet any of the retention requirements of the Entrance Scholarship
    Program, I am aware that I will no longer be a continuing scholar and I agree to pay the full
    tuition and miscellaneous fees of my enrollment for the Midyear, SY 2022--2023.
