AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, GINA ROSEMARIE R. CAMACHO and NICETA M. EVANGELISTA, both of legal age,
Filipinos, and residents of Tuguegarao City, Cagayan after having been duly sworn to in
accordance with law, hereby depose and state:
1. That we personally know the late EFREN GUMARU LOPEZ, who was married to
LUCIA Z. LOPEZ ;

2. That Spouses Efren G. Lopez and Lucia Z. Lopez have three(3) parcels of land, to wit:
a. “A parcel of land Lot No. 4212-B-2-b of the subd plan(LRC) Psd-158880,
being a portion of lot No.4212-B-2, LRC Rec. NO. 423, situated in Ugac Norte,
Tuguegarao City, Cagayan and covered by TCT No. T-97052.”

b. “A parcel of land Lot No. 4212-B-2-A of the subd plan(LRC) Psd-158880,

being a portion of lot No.4212-B-2, LRC Rec. NO. 423, situated in Ugac Norte,
Tuguegarao City, Cagayan and covered by TCT No. T-97053.”
and
c. “A parcel of land Lot No. 1351-B-1 of the subd plan Psd-2-01-035479, being a
portion of lot No.1351-B, situated in Ugac Sur, Tuguegarao City, Cagayan
and covered by TCT No. T-78936.”
3. That the above-mentioned lots are the conjugal properties of Efren G. Lopez and Lucia
Z. Lopez;

4. That Efren G. Lopez and Lucia Z. Lopez had three(3) children, namely:
a. Goodrich Z. Lopez;
b. Giovanni Z. Lopez; and,
c. Gervinna Z. Lopez-Garde.

5. That we are executing this affidavit to attest to the truth of the foregoing and for
whatever legal purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 15 th day of September 2023 in
Tuguegarao City, Cagayan.
