1. INTRODUCTION
1.1. Physical Location

ZUBAIR MISHRIF DGS EXPANSION is located in southern Iraq, 20 km southwest of Basra city

1.2. CIVIL WORKS IN EVAPORATION POND & MANIFOLD AREAS Overview

Site preparation and Earth works including survey, levelling, excavating and subgrade
compaction, HDPE liner installation, multiple backfilling, extra soil transportation, Foundations
of equipment, pipe rack, electrical sleepers, lighting poles, concrete pits and pavement works.
The construction activities include offloading from trucks, trailers, etc. of all equipment,
materials arriving to site, and relevant sub-contractor activities as contained in the scope.
1.3. Purpose

The purpose of this procedure is to define the process for rewarding good safety
behaviors and disciplining and managing violations with the intention to:

• To save lives.

• To modify worker and supervisor behavior by raising awareness of activities which


are most likely to result in fatalities and highlighting simple actions individuals can
take to protect themselves and others.
1.4. Scope of Application

This Plan is applicable to the Civil Works in Evaporation Pond and Manifold Areas of the
ZUBAIR MISHRIF DGS EXPANSION.

4. GOOD HSE PERFORMANCE

Through various initiatives, the following good HSE performance should be recognized such
as:

• Any out of the ordinary intervention by an individual.

• Outstanding periods of high safety performance by teams.

• Initiatives that have led to a demonstrable increase in safety awareness and/or


performance.

• Individuals or groups who have a track record of exemplary personal leadership in


safety through their actions and behaviors.

• Any other safety achievement that warrants recognition.


Good performance as shall be identified by the BBS program shall be recognized on a
monthly basis, and the reward for individuals shall be up to $30 or more cash gift while
group/team gift shall be up to $200 or more per period, which shall be either monthly or
quarterly.

5. BEHAVIOR-BASED SAFETY PROGRAM

A Behavior-Based safety program is a process through which work groups can identify,
measure and change their behaviors. The process applies the principles of the Antecedent
Behavior Consequence (ABC) behavior model. This assumes that all behaviors have one
or more antecedents or activators or prompts which initiate the behavior and one or more
consequences that either encourage or discourage repetition of the behavior.
5.1. Behavior-Based Safety Program Process Map

• QA shall set-up a Steering Committee (SC) for the BBS program. This shall include
the Project Manager, Construction Manager, HSE Manager, Admin Manager and
Construction Engineer.

• The SC shall Identify the behaviors critical to obtaining required safety performance.
Behaviors should be described as specifically as possible. Behavior descriptions
should meet the following criteria:
o Measurable - can be measured
o Active - something the worker has to do
o Reliable - the behavior is repeatable the same each time and at least two
people should be able to see the behavior and measure it the same way
o Controllable - the action is the control of the worker performing it
o Observable - can be observed, seen happening
o Specific - described so that the worker doing it knows exactly what to do.

• Communicate the behaviors and how they are performed correctly to all employees.
Communication shall be through meetings, pep talks etc.

• Observe the work force and record safe/at risk behaviors. Intervene with workers to
give positive reinforcement when safe behaviors are observed. Provide
coaching/correction when at risk behaviors are observed.

• Observations should be planned when possible and executed to include the


following:
o Do observation in a minimum of two persons team,
o Consider observing work where the higher risk hazards, or the experience of
the workers may be a factor,

o Avoid interfering with the work activities,
o Complete the observation report away from the work area,
o Examine the work area for access/egress, housekeeping.

• Collect, record and analyze observation data to prepare as basis for further
communication

• Communicate observation data and analysis results to all employee.


Communication to the work group can act as an antecedent in the ABC model. What
is not known cannot be corrected. It is expected that the simple act of
communicating the information will prompt the work group to proactively correct
their unsafe behaviors.

• Provide recognition or celebrate when safe behavior improvements occur. In this


step the appropriate recognition of the workers or celebrations happen when the
desired, or improvements in, behavior performance occurs.

• Change behaviors to be observed or change activators or change consequences


as appropriate

• Communicate any changes to the employees. Any changes made in the preceding
step, work force. If the changes are going to be made it is essential that all the work
force know what they are so they can change their behaviors accordingly.
5.2. HSE Violation

The following ZeTo violations shall always incur disciplinary action:


- Working without a valid work permit when required.
- Failure to adhere to rules of working at height
- Failure to verify energy isolation prior to starting work.
- Failure to obtain authorization before overriding or disabling safety critical
equipment.
- Entering a confined space without authorization.
- Failure to protect yourself against a fall when working at height.
- Refusal to use appropriate PPE while handling hazardous chemical
- Carrying out excavation or entering a trench without authorization.

- Walking under a suspended load
- Smoking outside designated smoking areas, and/or carrying or use of
unauthorized ignition sources in hazardous areas.
- Appearing at work under the influence of drugs or alcohol.
- Exceeding the speed limit

6. SAFETY INCENTIVE PROGRAM

Our incentive program shall be Behavior-Based Safety (BBS) Programs. They are not tied to
illness and injury rates, but to engaging in safe practices in the workplace. In this case,
rewards may be provided to workers who identify dangerous working conditions or
suggesting potential improvements to safety policies. In many cases, these programs also
penalize employees for not engaging in best safety practices.

7. VIOLATION MANAGEMENT IMPLEMENTATION


The disciplinary action shall consist of admonition, reprimand or dismissal, and be performed
accordingly. For appearing at work under the influence of drugs or alcohol or for violations that
lead, or could lead to disastrous consequences, a single violation shall lead to dismissal.
If the violation is due to Manager’s or Supervisor’s dereliction of duty, e.g. provision of a vehicle
not fitted with seatbelts, giving orders in conflict with standards or procedures, disciplinary
actions will apply to the Supervisor or Manager and not to the Employee or contractor.
High potential safety violations, that pose a severe risk to life, are considered cause for
immediate dismissal. These high potential safety violations include failure to 'hook on' during
work at height, smoking in production areas or any severe deliberate negligence that could
cause loss of life.
The violation management procedure shall be as follows:
• The violation report is received by the HSE department
• HSE Manager investigates or assigns someone to investigate
• If violator is confirmed to be without understanding of the rule violated – no evidence of
training received in that line, then violator shall be counselled and trained accordingly in the
rule
• If violator is confirmed to be informed of the broken rule, and he/she willfully violated (not
under compulsion by a superior’s instruction) he/she shall receive 1st written warning and
counselling

• If violator’s superior is confirmed to be the root cause of the employee’s violation, and
additionally confirmed that violator raised the rule to the superior who insisted on
employee’s action, the superior shall be issued a final written warning and counselling
• Refresher training in all ZeTo HSE rules shall be given to the violator and or superior
afterward
• For a 2nd time violator on the same rule or another ZeTo rule, he shall receive 2 weeks
suspension without pay from work
• Upon resumption violator shall receive refresher training in ZeTo HSE rules prior to starting
work
A 3rd violation shall lead to summary dismissal from the project.

9. RESPONSIBILITIES AND ACCOUNTABILITIES


HSE department has the responsibility to give first training on these ZeTo rules to all staff
during HSE Induction, which must be signed by every attendant.
Departmental/sectional heads and supervisors are accountable to remind and refresh their
personnel of these rules and adherence to such.
All employees and subcontractors also have accountabilities for themselves to obey and refresh
their minds of these rules and adherence to such.
9.1. Project Manager
• He provides every resource to make implementation of this procedure possible
• He appoints the Steering Committee of the BBS Program
• He presents the award for recognition of good HSE performance by employees
• He approves the disciplinary measures for any consequence management.
9.2. Construction Manager
• He ensures that this procedure is communicated to every employee and subcontractor
• He takes part as a member of the Steering Committee
• He ensures availability of every employee to be trained accordingly in this procedure
• He supports the project manager in recognition and discipline of personnel in line with
this procedure.
To achieve a safe site, all subcontractors, employees, supervisors and managers have the
responsibility to report violation of these rules to the HSE department so such that incidents
can be prevented and arrested early in the form of observations.
