The promotion of high levels of safety and health at work is the responsibility of the society as a
whole. All members of society must contribute to achieving this goal by ensuring that priority is
given to Occupational Safety and Health (OSH) in national agendas through building and
maintaining a national preventative safety and health culture. Good occupational safety and
health management and its practices will lead an organization to greater success.
b. Explain in sequence the factors that lead to an accident based on the Domino Theory of Accident.

It is worth repeating that poor standards of safety and health at work mean poor business. Every
year in Malaysia, a number of workers are killed or injured in industrial accidents or suffer from
occupational diseases, many of which go unreported or undiagnosed. In the past, any facts or
figures related to labour protection have often had little impact on decision‐makers as accidents
and ill health at work had always been seen as a natural consequence of production.
In your opinion, what are the reasons of not reporting any accidents that happened at the
workplace?

Risk management is the process of identifying OSH hazards, determining the risk based on
likelihood and severity of work injuries, ill health and property damage, prioritising the
implementation of preventive measures to mitigate these risks and communicating the risk
assessment to workers.
b. Explain the FIVE (5) principles of Hazard Identification Risk Assessment and Control
(HIRAC).

Workplace injuries cost employers billions in insurance claims, lost productivity, government
fines, and lawsuits every year. Poor workplace-safety practices also provide labour unions a
reason for being. Minimizing the risk of accidents begins with employees themselves and their
management team's absolute commitment to safety as a core value of the company. This means
employees need to do more than just set safety as a priority.
a. Discuss THREE (3) principles of accident prevention in ensuring the success of any accident
prevention programs conducted at the workplace.

b. Suggest and discuss ONE (1) training program that should be conducted for newly-hired
employees.

Hazard is the potential for harm or an adverse effect to people as health effects, to organizations
as property or equipment losses, or to the environment. As a newly appointed safety and health
officer for a paper manufacturing factory, you were asked by a safety and health manager of your
department to conduct the following task;
Identify the hazards associated with the steps of the paper manufacturing process. Propose the
control measure to mitigate for each of the hazards identified. You should also suggest the
appropriate training programmes to be given to the workers of this factory.
