Note 办公室第一份复印件---存放在 HSE 办公室
HSE Department reserve the right to REVOKE this PTW if there is any noncompliance's to the HSE Requirements
during sub-contractor performing their work. 当分包商在作业中出现任何不符合 HSE 要求的情况，HSE 部门保留
撤销此
PTW 的权利。
Applicant must copy and distribute this permit after part 4 is completed.
All the appropriate signatures must be obtained before work starts.
申请人必须在第四部分完成后复印并分发本作业许可证。
工作开始前必须签字完毕。
