AFFIDAVIT

I, KAMELLIE ANGELI E. GALARRITA, of legal age, single and a resident of

Zone 5, LANAO ALUBIJID, Misamis Oriental, after having duly sworn to in
accordance with law, hereby depose and state that:

1. I am an unmarried woman; (PSA issued Certificate of No Marriage, dated

June 22, 2023 is hereto attached and marked as Annex ‘A’.

2. I have a child/daughter named SAOIRSE ANDREA GALARRITA, born last

26 April 2016; (PSA issued Certificate of Live Birth is hereto attached and
marked as Annex ‘B’.

3. I am not cohabiting with a man, partner or co-parent and exercise sole

parental care and support of the aforementioned child; ( Barangay Affidavit,
dated July 6, 2023 is hereto attached and marked as Annex ‘C’.

4. I am executing this affidavit to support my application for the Solo Parent

Law, to attest the truth of the foregoing facts and for whatever legal
purpose it may serve.

IN WITNESS HEREOF, I have hereunto set my hand this 14th day of July 2023,
