PTO - Suspended Scaffolding- Installation, Dismantling and Relocation

CHECKPOINTS CHECK
1. Has suspended scaffold come from yard and been inspected?
2. Has the suspended scaffold been erected near overhead lines, electrical lines etc?

3. Trained/Authorized employees conducting work?

4. Employee undergone not scared of heights, filled in psychological questionnaire

5. Employee not under the influence of alcohol or drugs?

6. PPE: Overalls, gloves, safety footwear, safety harness, gloves?

7. Positioning of scaffolding suitable and safe, allows free safe access?

8. All material lowered and not thrown to lower levels?

9. Is harness being used? Fitted correctly and hooked onto fixed object during procedure?

10. Has area below dismantling been demarcated and signage erected?

11. Where used is the Lifeline suitable and strong enough to take the load of fallen person?

12. Are employees following the engineering designs and specifications?

13. Are visible employees on platform during dismantling and erection process?
14. Scaffolding material in good condition – no corrosion, bent, worn etc?
