1. Which part of Malaysia legislation for PTW in the work place.

FMA 1967- Safety ,health and welfare regulations -Section 13(2) : No person shall be required or permitted to enter any confined space Occupational Safety and Health Act 1994 -Section 15(1) : Employer and every self-employed person to ensure , safety and health of welfare at work of employees Section 15(2) : include provision and maintenance of plant and systems of work that are see and without risks to health Part VI, Section 24 : General duties employees care for safety and health of himself and other person affected by his acts or omissions at work 2. Procedure of getting PTW sign form by an AA

AA (Approval Authority) the person authorized by the OPU to approve and issue PTW in the area concerned. Work involving increased/unusual hazards (e.g. open fire , hot tapping , entry to confined space etc.) shall require review and authorization at a higher level in the organization . Planning -discussion involved 1. 2. 3. 4. Task to be performed . Tools and equipment required . The area system and equipment to be worked on . Potential hazards and control measures .

Permit preparation o The permit applicant must fill section 1&2 . o These sections are used to complete background information on location and the work required . o Hazards are identified in section 2. Document check Approving authority will review the permit and the associated . Ensure that the controls and precautions are fully specified. Supporting certificates are reviewed. If any missing , he initiates their preparation with the relevant disciplines. Section 3-5 to be completed by AA.

 All documentation are placed together and becomes a work pack. Work site preparation Approving authority will hand over the documentation to the approving authority representative to verify the worksite . Approving authority oversees the steps involved in preparing the worksite . Isolation integrity is verified by Competent Person & the Approving Authority Representative by starting the equipment from the local push button.

Electrical Isolation Before issuing a Work Permit , ensure that the equipment / facility is electrically safe and electrical power is isolated for the safe conduct of the authorized work. Issuing , Executing authorities and Electrical Maintenance shall jointly decide the extent of Isolation required. DO NOT OPERATE tag shall be used when isolation by local switch is decided as adequate protection . Multi-lock system to be used when different craft are involved (this is towards best safety practices)

Work permit Approval Approving authority signs to authorize permit to proceed work . Receiving Authority and Work Leader sign to accept the condition on the work permit .

3. How does PTW can control the access and the contractors at work site ? To secure the safety, health and welfare of persons at work against risks to safety or health arising out of the activities of persons at work To protect person at a place of work other than persons at work against risks to safety or health arising out of the activities of persons at work To promote an occupational environment for persons at work which is adapted to their physiological and psychological needs Outline how the work is to be undertaken (e.g. procedures, precautions, equipment, location , start time, duration) - verbally and where necessary in writing . Maintain records of work permits . Authorized permit issuers are to be competent with the implementation of the requirements of these guidelines. This may be attained by reading and familiarizing themselves with the requirements of these guidelines. Ensure that all tags and signs are prominently displayed so that personnel are aware that the equipment etc is isolated/not to be operated. Adhere to the PTW requirements . Ensure the job is performed in a safe manner Issuing/cancelling any apparatus interface statements with other operating authorities in liaison with their authorised officer. 4. You are asked to remove a desk from your class next door .How do you go about to get PTW sign ?

PLANNING is my first step to get the PTW sign , so I will discuss with my class members about Task to be performed ,The area system and equipment to be worked on , Potential hazards and control measures , Appropriate approval for the work and All person in charge are aware of the interaction with other work activity. PERMIT PREPARATION . The students involve in the work including me will fill section 1 and 2 in the PTW application form . Actually ,These sections are used to complete background information on location and the work required and then the hazards are identified in section 2 .

 DOCUMENT CHECK will be the next , here the Approving Authority will review the permit and the associated ,ensure that the controls and precautions are fully specified , supporting certificates are reviewed , preparation with the relevant disciplines , section 3-5 to be completed by AA so all documentation are placed together and becomes a work pack . WORKSITE PREPARATION , AA will hand over the documentation to the AAR to verify the worksite and the steps involved in preparing the worksite . WORK PERMIT ACCEPTANCE/APPROVAL , AA signs to authorize permit to proceed work followed by RA and Work Leader sign to accept the condition on the work permit . WORK PERMIT DISTRIBUTION where the Copies of work permit goes to : Worksite(original), Control Room or Permit Office(no. 1), Receiving Authority(no. 2). The Work Supervisor (class leader) is responsible to display the copy of the work permit at worksite. TOOLBOX TALK is held to discuss about the , planned activities and work followed by Hazards, potential risks and control. WORK SUSPENDED is carried out if Any responsible member of Prod. dept. or HSE is authorized to suspend work and withdraw the Work Permit if, safety regulations are sullied. If work is not started or is stopped for over two hours, the permit shall be revalidated by the issuing authority . WORK REVALIDATION is the next . here work permit shall be initially issued for a period of 12 hours and shall be renewed or revalidate at the start of each shift at the permit office . WORK COMPLETION , Work leader shall return the work permit to the AA and to Ensure the worksite is left in a clean and safe condition . HANDBACK section 8 is completed . ARCHIVE , where the copies and the closed permits shall be properly filed and kept with the respective disciplines for at least one month from the date of issue/closure. Different types of permits shall be filed separately in chronological order in box files.

5. 3 examples of certificates use in PTW and explain under which situation those certificate are required.

Mechanical Isolation (GREEN/WHITE) For process and mechanical isolation lock and tags are to be used

Electrical Isolation (YELLOW) For work on electrical equipment and installations, or on equipment that is connected to same, all power supply to the equipment/installation shall be isolated/disconnected electrically. for work on electrical equipment and electrical driven equipment

Confined Space (BLUE/WHITE) Preparation for entry personnel to do work

Gas Test (RED/WHITE) Check for safe atmosphere by trained gas testers
