Health Checkup Period

April 1, 2023 – March 31, 2024

Eligible Courses

The eligible course for health checkup differs depending on your age as of March 31,
2024. Please click here to see the test items on each examination and the respective
course type.
Under 35 years old: Basic checkup
Age 35 and over: Basic checkup, Specific checkup, or Comprehensive checkup

Payment and subsidy

From April 1, 2023, we have some changes in the amount of subsidy for optional tests and
items of optional tests eligible for subsidies. Please check this guide carefully.
- This subsidy is for employees only and not applicable for dependents.
- Please note that the subsidy amount remains unchanged if you choose medical institutions
other than ITS (Kanto IT health Insurance Union)’s health checkup centers and its
contracted clinics.

 Subsidy for optional tests

- Subsidy amount: Under age 35 JPY6,000/ Age 35 and above JPY10,000

- Test items eligible for subsidy from the company is being updated from Apr 2023. Please
refer the table below for more details.
- ITS- specified gynecological tests(breast / uterine cancer screening) will be subsidized
separately.
- Some clinics may not offer any of the tests below.
- Please ask clinic to fill in the test items on the receipt.
- You will be responsible to pay any extra amount that exceeds the ceiling.

Gynecological optional tests are fully subsidized separately from the optional tests’ subsidy.
Only ITS- specified gynecological tests are covered. Please ask clinic to fill in the test items
on the receipt.

– No need to pay
– Can be booked on the website. English questionnaire is available. Please enter your
home address as the shipping address for the test kit.
Sapia Tower Clinic https://sapiatower-clinic.jp/contact/
– No need to pay (if the amount exceeds the ceiling, it will be deducted from the salary)
– No online booking. Please call 03-5288-0610.
– English speaking doctor is available on Thursdays.

Clinics – Expense reimbursement required
Other ITS-contracted Clinics
– Need to pay first and reimburse it as expense.
– The type of course and optional check-up items based on the clinic.
– Make an appointment to clinic by phone directly. Please ensure to tell them that
you will pay the checkup fee on-site and request not to send a bill to Capgemini.
– English Service is available at Tokyo Midtown Clinic.

<How to Claim Expense>

If you took health checkup at any of ITS-contracted clinics (except Sakakibara Sapia Tower
Clinic), you need to claim the expense reimbursement via Concur. Please note that the
project code differs depending on the Market Unit to which you belong. If you’re not sure
your Market Unit, please contact DL JP HR hr.jp@capgemini.com for help.

< Timesheet>

Clarity: For the time you spent for health checkup, please enter the actual hours spent
using your project ID and as Non-Billable.

Workcloud: please enter the hours as working hours in timesheet and mention in the notes
section the time you were away for the health checkup.

Please return to work immediately after completing the health checkup.
