AFFIDAVIT OF DENIAL

I, ARIES, of legal age, and a resident of Barangay Talaga, Capas,

Tarlac, after having been duly sworn to in accordance with law, depose and
state, that:

1. I am applying for an NBI Clearance in Tarlac City;

2. However, upon processing said clearance, I learned that according

to the records of the Office of the Prosecutor of Tarlac City, a
person named as Aries N. is a respondent in a case of Violation of
Art. 315 (Estafa) with NPS Docket No. III-INV-17G;

3. I have not been charged of the above-mentioned criminal offense,

and I therefore hereby declare that I am NOT the same person
named in the above-mentioned criminal case;

4. I am executing this affidavit to apprise all those concerned of the

foregoing facts and for whatever legal purpose it may serve.
