On the day 12/4 2013 G.C the store keeper obtained from the previous bin card the
result for Mebendazol 100mg/5ml suspicion of 60ml of bottle were 15,000 with
Expired date of December 2014 and batch number 1555.
1. On the day 13/4/2014 G.C, 20.000 bottles Mebendazol 100mg/5ml suspicion
with expired date september2013 and batch number 3235 donated from USAID.
2. On day 16/14/2013 G.C, 2500 bottles of mebendazol 100mg/5ml suspicion
were issued to Shashemena health center.
3. On day 22/4/2013 G.C, 7000 bottle of Mebendazol 100mg /5ml were
transferred to Kore health center and 5000 were loaned to Tigray health center
4. On the day 28/4/ 2013 G.C, 4000 bottles of mebendazol 100mg/5ml
suspension were issued to dispensary and the store keeper discover 500 bottle s
with batch number 3235 were broken.
5. On the day 3/5/2013 G.C, 2500 bottles of mebendazol 100mg/5ml suspension
were Loaned Tikur Anbesa hospital and 2000 were issued to pediatric pharmacy.
6. On the 7/5/2013 G.C PFSA supply 2000 bottles of Mebendozol100mg/5ml
suspicion with a batch number 1245 Expired date March. 2013 from which 50 was
broken.
7. On the day 10/ 5/ 2013 G. C, 3000 bottles of Mebendozol100mg/5ml suspicion
were transfer for both Adama hospital and Bisheftu Hospital respectively.
8. On the day 15/5/ 20013G.C, 4500 bottles of Mebendozol100mg/5ml suspicion
were issued to Kirkose health center and the store keeper identified 600 bottles
were broken.
9. On the day 30/ 5/2013 G.C physical inventory was conducted and 1700 was
obtained. The reason for discrepancy is unknown.
