the following identity verification documents to process your request.

Instructions For Gathering Required Documentation

Gather ONE (1) form of identification from the list below.

Primary types of Identification (ID) (Only one required):

Motor vehicle operator's license. A Florida Driver's license may be expired up to one year and will be
accepted if legible and intact
Motor vehicle state identifcation card
Learner's/Instruction Driver's Permit, if over 18 years of age
Passport - must be signed
Military ID, DD Form 2, Military dependent ID, active or retired, DD1173
Resident Alien Card, Form 1-551(Permanent Resident Card) - ID card issued by the U.S. Dept. of
Justice - Immigration & Naturalization, Form 1-97
Certification of Naturalization
Inmate ID card issued on or after January 1, 1991 by Florida Dept. of Corrections (DOC), (Inmate still
in custody of DOC)
Inmate ID card issued by U.S. Dept. of Justice (DOJ), (Inmate still in custody of DOJ)
If just released from penal institution, client should have a letter from the institution or prison ID
Pilot's License
Concealed Weapons License

FOREIGN DRIVERS LICENSES ARE NOT ACCEPTABLE AS PRIMARY ID

Instructions For Submitting Your Documentation

Using a DIGITAL CAMERA or CAMERA PHONE

1. Take a picture of each document that you gathered in the steps on

the previous page.
2. Be sure the following information is visible on digital images of
identification: Full name and address, signature, license or
identification number, and expiration date
3. Upload or email the image(s) to your computer
4. Visit the 'Manage My Order' page on VitalChek.com:

5. Enter your order number: 169850379 and PIN: 200975
6. Select 'Upload Documents Now' under #2 on the 'Completing Your
Order' page
7. Select the uploaded image file(s) that you wish to upload
