Checklist

Shift Handover

Structured log book or hand over checklist is used for shift hand over

Shift handover is carried out at designated location for all shift personnel.

Shift handover is carried out face to face before end of shift using Shift Logs.
For Shift Leader handover the respective Shift Activity Logs (for offshore including Crew Change
Handover Note) are reviewed and sign-off together between in-coming and out-going shifts

- HSE issues that are relevant to the work carried out during the shift.

- HSE incidents, including near misses.

- Deviation from expected process and equipment performance affecting the process and equipment
integrity.

- Excursion beyond safe operating limits (SOL).

- Planned jobs, hazardous/ non routine operations carried out during the shift.

- Safety Critical Protective Device (SCPD) bypassed.

- Urgent or critical alarm activation that needs follow up

- Lockout tag-out (LOTO) status.

- Production plan and logistics.

- Shift job status and PTW extensions, especially jobs with hot work permits, confined space entries
and other critical activities

- Product quality issues.

If there is shortage of manpower (i.e. below the minimum manning level) for the incoming shift, the
corresponding outgoing shift member is not allowed to leave until a suitable replacement arrives at
site.

At the beginning of every shift, the shift leader conducts a briefing that is attended by all incoming shift
personnel. He shall ensure,

• Minimum shift manning is met

• Shift hand over done

• All items discussed in the shift handover is communicated

All members of the ERT are identified at the beginning of the shift.

The names of the ERT are clearly displayed at strategic location
