13 Areas of Assessment Assessment

Assessment of the patient's mental health,


I. Psychosocial and Psychological Status emotional state, and social support systems.
II. Mental and Emotional Status Assessment of the patient's mental health,
including mood, affect, and cognitive functioning.
III. Environment Status Assessment of the patient's living and working
environment for factors that may affect their
health.
IV. Sensory Status The assessment of the patient's ability to perceive
their environment through their senses.

A. Visual status: Visual acuity and ability to see


objects clearly.
B. Auditory status: Ability to hear sounds and
perceive auditory stimuli.
C. Olfactory status: Sense of smell and ability to
detect different odors.
D. Gustatory status: Sense of taste and ability to
differentiate between different flavors.
E. Tactile status: Sense of touch and ability to
perceive tactile stimuli.
V. Motor Status Assessment of the motor functioning, patient's
ability to move and control their body, including
balance, range of motion, strength, coordination,
and mobility.
VI. THERMOREGULATORY STATUS Assessment of the patient's ability to maintain
their body temperature within a normal range.
VII. RESPIRATORY STATUS The assessment of the patient's lung’s function
and breathing patterns, including lung sounds
and oxygen saturation.
VIII. CIRCULATORY STATUS The assessment of the patient's heart and blood
vessels, including blood pressure, heart rate, and
circulation.
IX. NUTRITIONAL STATUS Assessment of the patient's nutritional status,
including dietary habits, weight, and laboratory
values.
X. ELIMINATION STATUS The assessment of the patient's lung’s function
and breathing patterns, including lung sounds
and oxygen saturation.
XI. SLEEP, REST AND COMFORT STATUS The assessment of the patient's sleep patterns,
restfulness, and comfort levels.
XII. FLUIDS AND ELECTROLYTES STATUS Assessment of the patient's fluid balance and
electrolyte levels, including intake and output
measurements.
XIII. INTEGUMENTARY STATUS Assessment of the patient's skin for any
abnormalities, including rashes, lesions, or
wounds.
