JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

(Re: Late Registration of Birth)

We, JUDITH C. AHON and SHERYL A. TORREGOSA, Filipinos, of legal ages, both
married and residents of Purok 7, Salvador, Cortes, Bohol, Philippines, after being sworn in
accordance with law, hereby depose and say:

1. That we know personally the person of DONNA KAYE PAÑA who was born on August
30, 2010, at Cortes, Bohol, Philippines, being the daughter of Dominga S. Paña because
we are neighbors thereof;

2. That the fact of birth of Donna Kaye Paña was not recorded in the Local Civil Registrar
(LCR) of Cortes, Bohol due to inadvertence and excusable negligence;

3. That we are not related to the said person and we are not interested in whatever outcome
this presents may bring;

4. That affiants further attest to the truth of the foregoing statements for whatever other
legal purpose this may serve.
