I, ELVIRA B. YLAYA, of legal age, resident of Andres Abellana, Ext. St.

Guadalupe Cebu City, after being sworn to in accordance with the law do hereby deposed
and say:

1. That I am the registered owner of 4 – UNITS RESIDENTIAL located

Andres Abellana, Ext. St. Guadalupe Cebu City, with Building Permit No.
BO12019500173 dated March 06, 2019.

2. That the electrical plans and specifications was prepared and signed by Eng’r. Antonio B.
Daclan when submitted to OBO Cebu City.

3. That during the construction Eng’r. A. Daclan did not supervise the electrical work. For
this reason I hire the services of Eng’r. Dexter Cañeda for the electrical supervision and
completion application signature.

4. That this affidavit is executed to attest to the truth of the foregoing facts an required by
OBO for the issuance of Certificate of Occupancy.
