َّ ‫)ال ِعلَّوص هو نقص مؤقت في التقلُّصات ال‬
)‫طبيعيَّة لعضالت األمعاء‬

Def: failure of transmission of peristaltic waves due to neuromuscular failure

The resultant stasis result in accumulation of gas and fluid within the bowel, with
associated distension, vomiting, absence of bowel sounds and absolute constipation
‫عصبي‬-‫فشل في نقل حركة تقلصات االمعاء بسبب خلل عضلي‬
‫ مما يسبب توسع األمعاء‬،‫وهذا التوقف الحاصل في حركة األمعاء يؤدي الى تجمع السوائل والغازات داخل األمعاء‬
‫والتقيؤ وانعدام أصوات األمعاء واالمساك‬
Causes ➔ / Postoperative ‫ → ما بعد العملية‬self-limiting with a duration of 24-72 hours
Postoperative ileus maybe prolonged in the presence of hypoproteinemia or
metabolic abnormality

/ Infection ‫ → التهاب‬intra-abdominal sepsis

/ Reflex ileus → may occur following fractures of spine or ribs ‫يحدث كرد فعل بعد حالة‬
‫معينة‬
/ Metabolic ‫ → خلل ايضي‬uremia and hypokalemia (most common)

Irritation to bowel lead to:

Sympathetic activation → decreased or arrest of peristalsis

Local release of nitric oxide → relaxation of smooth muscle → decrease or arrest

of peristalsis

Clinical features ➔ / paralytic ileus takes on a clinical significance if, 72 hours after
laparotomy: 1) no return of bowel sounds 2) no passage of flatus
‫ ساعة بعد العملية ما رجعن أصوات األمعاء الطبيعية او ما طلعت أي غازات‬72 ‫نبدي نفكر بهاي الحالة إذا ورة‬
‫من األمعاء‬
/ Abdominal distension → which increases pain from abdominal wound
)‫توسع (انتفاخ) البطن والذي يؤدي الى زيادة األلم من جرح البطن (جرح العملية‬
/ Vomiting may occur ‫قد تحصل عملية تقيؤ‬
Diagnosis ➔ / Clinical dx )‫تشخيص سريري (عن طريق االعراض والتاريخ المرضي‬
/ Radiologically → gas-filled loops of intestine with multiple fluid levels
Treatment ➔ / Nasogastric suction (decompression and relieve of abdominal
distension) ‫تتخلص من التوسع او االنتفاخ البطني عن طريق اخالء األمعاء من خالل أنبوب‬
/ Restriction of oral intake until bowel sounds and passage ‫اضغط هنا مقطع ممكن يفيدك حتى تفهم أكثر‬

of flatus return ‫تمنع أي ادخال فموي لحد ما ترجع أصوات االمعاء وتتم عملية مرور الغازات وخروجها‬
/ Fluid and electrolytes balance ‫تنتبه على مستويات السوائل واالليكتروليت وتصححها‬
/ Treat the primary cause when identified ‫اهم شي تعالج السبب الرئيسي بعد ايجاده‬

If paralytic ileus is prolonged CT scan is the most effective investigation; it can

demonstrate any intra-abdominal sepsis or mechanical obstruction

‫اذا طولت هيج حاالت وما انحلت فممكن نسوي اشعة مقطعية (مفراس) للبطن ونشوف في حالة اذا اكو التهابات داخل البطن او انسداد ميكانيكي‬

The need for laparotomy becomes increasingly likely the longer the bowel inactivity persists,
particularly if it lasts more than 7 days or if bowel activity recommences following surgery
and stops again

‫الحاجة لعملية فتح البطن مرة أخرى تزداد إذا استمرت عدم فعالية وعمل األمعاء وخصوصا ً إذا استمرت لسبعة أيام او إذا عادت فعالية األمعاء‬
ً ‫بعد العملية الجراحية ثم توفقت مجددا‬
