Greetings! This informed consent form is for Grade 12 ABM students of Arellano University and who we
are inviting to participate in this research study, titled "Aloe Vera as an Alternative Moisturizer of Grade
12 ABM Students from Arellano University Jose Abad Santos Campus".

You are being asked to take part in a research study. Before you decide to participate in this study, it is
important that you understand why the research is being done and what it will involve. Please read the
following information carefully. Please ask the researcher if there is anything that is not clear or if you
need more information.

The purpose of this study is to enlighten everybody's mind and give additional information that was
unheard of. We want to learn about the Aloe Vera Gel as an Alternative Moisturizer for students like you.
We believe that you can help us by telling us what you know and think. This knowledge might help us to
conduct and accomplish the study in order to solve the subject's issues and make everyone aware of its
benefits. There will be no direct benefit to you for your participation in this study. However, we hope that
the information obtained from this study may greatly benefit students, teachers, administrators, future
researchers, and individuals in our community. Your participation in this research is entirely voluntary. It
is your choice whether to participate or not.

We are inviting you to take part in this research study. If you accept, you will be asked to:

You may decline to answer any or all questions and you may terminate your involvement at any time if
you choose. Participant data will be kept confidential except in cases where the researcher is legally
obligated to report specific incidents. If you decide to take part in this study, you will be asked to sign a
consent form. After you sign the consent form, you are still free to withdraw at any time and without
giving a reason. Withdrawing from this study will not affect the relationship you have, if any, with the
researcher. If you withdraw from the study before data collection is completed, your data will be returned
to you or destroyed.
