NURSING CARE PLAN

•Anursing care plan (NCP) is a formal process that correctly


identifies existing needs and recognizes a client’s potential
needs or risks. Care plans provide a way of communication
among nurses, their patients, and other healthcare providers
to achieve healthcare outcomes. Without the nursing care
planning process, the quality and consistency of patient care
would be lost.

• Baby Renz is a newly born baby with a mother who is an adolescent. She was
transferred from the DR to NICU Level 1. You, as the student nurse on duty, placed
the newborn under a droplight and assessed the newborn through checking vital
signs and observation. The vital signs were: RR 75 cpm, CR 150 bpm, temperature
35.7 ̊C, and O2 saturation of 94%. With chest retractions, nasal flaring, and
grunting. The NICU nurse administered oxygen support via a face mask at 5 lpm
and initiated an IVF of D5 Water 38 cc at 4-5 ugtt per minute per the doctor’s
order. Antibiotics such as Gentamicin 14 mg IV and Penicillin-Tazobactam 175 mg
were administered as prescribed. For CBC, urinalysis, blood culture, sensitivity
test, and chest X-ray. The neonatologist diagnosed her with respiratory distress
syndrome.
