Dear Sir Hernandez.

Good afternoon and we apologize if we are ever interrupting your time.

We are the Staffs/Officers for the Organization named Sports Club and is in need of your permission
to let the students who participated in joining the Sports Club to have a Program every Week of
4:00pm – 5:00pm.

What is the name of our Program? The name of our Program is called Game Time specifically for
Sports Club and we are hosting our Program every Monday (Set A) – Tuesday (Set b) per Week.

We are also in need of your permission to let us Staffs/Officers having solicitation of (Cash, Sports
Materials) from the students who are part of our organization to be used for our School’s Sports
Club.

We are also not requiring an specific amount of cash from our members, as any amount of donations
are much appreciated as long as it helps our organization. The amount of Cash donations we receive
will be used to purchase Sports Materials for their practices and events. The Sports Materials will
also not be kept by the members and will be stored in our School.

Our Program’s Objectives:

1.) To reduce an Individual’s stress as well as to avoid their bad habits.

2.) To make us Individuals Physically fit and improve our Sportsmanship.

3.) To make each Individual’s teamwork better and by having Camaraderie.

4.) To enjoy our time at the end of our School’s Classes and have fun.
