ON-THE-JOB TRAINING WAIVER

I, Franz Eugene A. Sarmiento of legal age and residing at Blk. 5 lot 4 Jodimarey
Village, Sirang Lupa, Calamba City, Laguna accepted for OJT training program to by
freely and voluntarily assume and impose upon myself the following duties:

1) That I recognize the right of my OJT training site in which I may be placed and submit
myself to all the rules and regulations that may be imposed upon about my training,
and
2) That I waive my claim against that OJT site and the St. Vincent College of Cabuyao, for
any injury that I may bear in the performance of my duties and functions.
