Dear Ms Coyne
RE: Mr Kenneth Mason DOB: 27/02/2000

I am writing to request further treatment for this patient, an 18-year-old university student,
who was diagnosed with a chest pneumothorax today.

Mr Mason was transferred to our department suffering a chest contusion following football
practice with chest pain and breathing difficulties. He is active and does not have any medical
history related to these symptoms. Shortness of breath, low oxygen saturation of 92% and
peripheral bluish chest discoloration were observed. In addition, he was alert and oriented;
however, he was not able to complete sentences because of breathing difficulties. A chest X-
ray was performed, and this diagnosis was confirmed with him.

In terms of current treatment, an ICC was immediately administered, connected to water-


sealed drainage. This patient also requires monitoring of any bleeding in the ICC. 24 hours IV
fluid and 10 mg of Morphine (IV) were also administered. He may start free fluids and light
meals.

It would be appreciated if you could provide further care for Mr Mason such as monitoring his
vital signs. Also, please discuss with the nurse about his past medical history, current situation
and further treatment plans if necessary.

should you require further information.
