I visited sites and conducted a visual inspection then talked with workers, supervisor, engineer,
and other few professionals in health and safety management field. Their knowledge about the
proper risk assessment has shared with me. At the construction site I interviewed several
workers, engineer, project manager who had several years of experience behind them. they
provided the best valuable safety practices they have acquired through the life time.

I have reviewed records such as HSE plan, HSE policy, audit report, incident log book, accident
records, training records, first aid reports, near miss reports etc. and found that 8 months ago the
last accident occurred and found that there was no last time injury.

Part 3: Prioritise 3 actions with justification for the selection
Suggested word counts
Moral, general legal and financial arguments for all actions: 300 to 350 words
For EACH action:
Specific legal arguments: 100 to 150 words
Likelihood AND severity: 75 to 150 words
How effective the action is likely to be in controlling the risk: 100 to 150 words

Moral, general legal and financial arguments for ALL actions

Moral, general legal and financial MORAL
arguments The company has a moral duty to protect all workers who work either in the workshop or all the work site. RK
GROUP is a fast growing company with growing number of projects and employees from different places.
maintain safe work place and protect workers from hazards are companies one of major concern every
employee would like to return home from work place safely. any accident will cause the reputation of
company and moral values of workers and co- workers. An accident will affect the life quality of worker and
his family. Decrease in moral value in workers will affect the production also. Company consider the workers
are main pillars for business growth ;so they are giving a priority to providing a safe work place and
enhancing moral value of workplace. In addition to that morality of the workers obviously linked to the
condition of the work place, morality of the workers will be reduced significantly. As a result of reduction in the
workers morale, quality and productivity of the business will be affected adversely.so the company has to
provide reasonably practical control measures in order to protect the workers.
FINANCIAL ARGUMENTS
When an accident occured or any worker becomes sick due to the workplace exposure, the company has to
face huge financial loss in terms of direct cost and indirect cost.
 THE DIRECT COST : direct or insured costs for accidents are usually considered those costs
covered by workers compensation insurance and other minor medical costs for the accident .The
company pays insurance to cover these costs which will include all the medical expenses,
compensation to the workers and public the cost for the fine and enforcement, the cost the material
damage and property of damage and the cost to buy new equipment
 THE INDIRECT COST : Indirect costs are all the “uninsured” additional costs associated with an
accident. What is important to realize is that indirect costs are usually much greater than direct costs
which include the damage to reputation to the company
workers attention rate, loss of present and future projects, training and recruitment of new employee

Learner number:00735254 Learner name:Akhilraj A Page 12 of 17

cost, the down time cost of many related activities, investigation cost so on. The cost when compared
to implement the said safety precaution is way lesser than the direct and indirect cost combined and
in the long run the many human lives will be saved.
Here, most of the losses are uninsured costs. In other hands, by implementing recommended control
measures, company can avoid the accidents and ill- health and these costs would have become the profit of
organisation.
LEGAL ARGUMENTS
Employer has a legal duty to provide safe work ,safe equipment and material as per the ILO code ILO
convention 155(c155),ILO recommendation (r164). Employer should comply with legal and laws regulations.
implementation of legal practices is necessary for all organisation. there are some rules are set by
international bodies, if organisation fails to achieve those standards then enforcement agencies take further
action. At first they aware the organisation through general notice after that they take further steps. They can
stop the work which have potential to harm the employees. Enforcement authorities can also demand to start
the working after making certain improvements or taking respective control measure in order to make the risk
factor low.in case of any severe accident.
Overseeing chief can be imprisoned and firm license can be cancelled.

Justification for action 1

Action (Taken from column 4 of risk Provide scaffold tie like (box, tie, or through tie)for support scaffold(hazard category:work at height)
assessment)

Specific legal arguments R175-Safety and health in construction recommendation scaffold “every scaffold should be properly
designed, erected and maintained so as to prevent collapse or accidental displacement when
properly used”. So as the employer has to ensure above legislation for safe use of scaffold but if the
tie is not provided to the scaffold it will leads to scaffold collapse and it will also leads to
enforcement action following accident due to failure of follow of these legislation.
Consideration of likelihood AND severity The likelihood of injuries occurring from work at height especially on scaffolding is to be considered
as very high because more than 4scaffold are used in the particular site and almost 20 workers are
 types of injury or ill health working on the top and bottom of scaffold for masonry work and plastering work, so the scaffold
 number of workers at risk may shake due to movement of people and materials and weight of the materials in the top of the
 how often the activity is carried scaffold increase the risk of scaffold collapse. abscence of scaffold tie decreases the stability of
out scaffold so it increase the chance of scaffold collapse.
 how widespread the risk is
When considering the severity I set 4 categories:

Learner number:00735254 Learner name:Akhilraj A Page 13 of 17

 Minimal- no injury and/or damage occurred
 Minor- injury requiring first aid and/or slight damage for plant or equipment
 Major- injury requiring hospital treatment and/or major damage to buildings
 Catastrophic- death and/or irreplaceable damage to plant/equipment

In case of scaffold collapse it leads to fall of people from the top of scaffold and the people
or material falls to workers in the bottom workers. when person fall from top of scaffold
leads to severe cut injury or fracture some case death also. The bottom workers may also
injured like head injury and fracture. So the severity level is considered as catastrophic.
How effective the action is likely to be in
By providing scaffold tie it will enhance lateral stability and ensure a content and correct spacing
controlling the risk. This should include:
between the work surface and scaffold. Ties used to link the scaffold to any existing building or
 the intended impact of the action; structure. It provides resistance to inward and outward movements to the scaffold. Ties should be in
 justification for the timescale that such a way that it provides additional restraint to the scaffold. The provision of scaffold tie reduces
you indicated in your risk the likelihood of accident so the risk will reduce considerably.
assessment; and I had recommended two weeks for complete this action because the project manager needs to
 whether you think the action will arrange sufficient coupler and scaffolder for complete this work
fully control the risk
This action alone won’t control the risk fully, In addition with implement scaffold tag system to
ensure safe scaffold, Conduct third party inspection body approved by Hyderabad municipality
those scaffold above 10m will control the risk as low as reasonably practicable level.

Justification for action 2

Action (Taken from column 4 of risk Purchase and fix the crown guard on the circular saw machine( hazard category: work equipment)
assessment)

Specific legal arguments According to ILO C-199 guarding of machinery convention,1963 (No.119),Article 11 states that “no
worker shall use any machinery with out the guards provided being in position, nor shall any worker
be required to use any machinery without the guards provided being in position”. So the employer
has to ensure above legislation for safe use of circular saw but if the guard is not provided to the
circular saw it will leads to accident and it will also leads to enforcement action following accident
due to failure of follow of these legislation.

Learner number:00735254 Learner name:Akhilraj A Page 14 of 17

Consideration of likelihood AND severity The likelihood of injuries occurring during usage of circular saw is considered as veryhigh
especially without guard system because the circular saw is continuously use in this particular site
 types of injury or ill health almost 5workers working 7 hours in a day. A table saw cuts wood with a circular blade whose teeth
 number of workers at risk are moving, in the direction of the operator, at over one hundred miles per hour these create
 how often the activity is carried several hazards: they can cut fingers and hands due to accidental contact with saw, they can lift up
out and throw a work piece back at the operator at high speeds(“kickbacks”), due to absence of crown
 how widespread the risk is guard.
Please see `justification’ 1 for severity categories. The severity of the risk occurring could be fairly
serious are likely to range from minor injuries such as bruising, cut injury, slightly more serious like
amputation or eye injury or loss of vision due to kickback.so the severity is major
How effective the action is likely to be in
A guard should prevent employees from contacting the dangerous parts of the machines, and it
controlling the risk. This should include:
should be secure. implementation of safeguard reduce accidental contact with rotating blade. A
 the intended impact of the action; plastic barrier guard is suspended over the blade to prevent inadvertent blade contact from above,
 justification for the timescale that the sides and the rear. It also prevents the blade’s rising teeth from throwing back the work piece
you indicated in your risk towards the operator. The provision of guard reduce the likelihood of accident, so the risk reduce
assessment; and considerably.
 whether you think the action will I had given 3 weeks to complete this action because the employer needs to purchase the guard
fully control the risk system from the supplier and fix it in table saw. This action alone won’t control the risk fully, In
addition with provision of lighting in the working area and changing of blades with broken teeth it
should fully control the risk.

Justification for action 3

Action (Taken from column 4 of risk Purchase radio communication like walkie-talkie for use between the safety watch, confined space
assessment) entrant and the supervisor (hazard category: Confined space)

Specific legal arguments The confined spaces Regulations 1997 work in confined spaces states that “no person at work shall
enter a confined space to carry out work for any purpose unless”. It is not reasonably practicable to
achieve that purpose without such entry.
C167 – Safety and Health in Construction Convention Article 13 SAFETY OF WORKPLACES
states that ”all appropriate precautions shall be taken to ensure that all workplaces are safe and
without risk of injury to the safety and health of workers. ”so the employer has to ensure above
legislation for proper communication between confined space entrant and safety watch. If the
walkie-talkie is not provided and it will also leads to accident in case of worker feel any sudden

Learner number:00735254 Learner name:Akhilraj A Page 15 of 17

illness and also leads to enforcement action due to failure of follow of these legislation.
Consideration of likelihood AND severity The likelihood of illness or injuries occurring from confined space is very high because in the site
many confined space activity is going on daily basis. almost 5 workers working in the confined
 types of injury or ill health space also increase the chance of hazard. The confined space have deficiency of oxygen
 number of workers at risk atmosphere so the absence of radio communication like walkie-talkie in confined spacy delay the
 how often the activity is carried evacuation process in case of emergency increase the chance of risk
out
Please see ‘justification’1 for severity categories. The severity is also catastrophic
 how widespread the risk is
Because the communication gap between the fire watch and workers inside the confined space
makes delay in rescue activities it will leads to death of workers.
How effective the action is likely to be in
As workers may not be able to see each other if one is inside in a confined space and the other is
controlling the risk. This should include:
outside, a direction line of communication using walkie-talkies or another device will help maintain
 the intended impact of the action; contact even if supervisors cannot keep them in their line of sight. Using walkie-talkies can alert the
 justification for the timescale that staff member outside of the enclosed space if someone is in need of rescue or emergency medical
you indicated in your risk services in case things go wrong.so they can save the life of the person. Other communications
assessment; and equipment can include cellphones, however, the conditions of an enclosed space may make it
 whether you think the action will difficult for phones to receive a signal
fully control the risk I had given one month for complete this action is enough for the employer to purchase the walkie-
talkie from the supplier and needs to provide training for the workers about how to use.
This action alone won’t control the risk fully in addition with provision of periodic training to the
confined space employees and practice of evacuation drills will control the risk fully

Part 4: Review, communicate and check

Suggested word counts for each section:
 Planned review date or period and reasoning for this: 50 - 100 words
 How the risk assessment findings will be communicated and who needs to know the information: 100 - 150 words
 Follow up on the risk assessment: 100 - 150 words.
Planned review date/period with As per company policy the review of the risk assessment has to be done at every 12 months. Therefore I
reasoning given the review date for 12 months timescales 23/01/2024.But, in case any major accidents change in
management or policy happens inside the organisation, the risk assessment should be reviewed according
to the change.
How the risk assessment findings will be I will arrange a conference meeting with the management to discuss the risk assessment and how

communicated AND who you need to tell the additional control measures can be effectively implemented in future . I will also point out the
management team about the statutory requirement like ILO guidelines, hyderabad muncipality
guide lines followed in construction site. I will take all the actions to include the findings of risk
assessment in the next available tool box talk which have to be discussed with the workers. I will
also publish the summary of risk assessment in company website and notice board in site so any
one can easily access.
How you will follow up on the risk After doing the risk assessment I will set up a reminder for 5 days whether they have taken any
assessment to check that the actions action for completing the work as per assessment report. Then I will put up E-Mail to the
