AFFIDAVIT OF RECOVERY OF
TRANSFER CERTIFICATE OF TITLE

I, FRANCISCO M. CASTRO, JR. of legal age, married, Filipino, with residence and
postal address at Longos, Pulilan, Bulacan, after having been duly sworn to in accordance with
law, do hereby depose and say:

That I am the lawful owner of a certain parcel of land located at San Jose, Plaridel,
Bulacan with an area of 487 Square Meters, more or less, and covered by TCT No. T-108227 in
the name of the late Rosario Lopez Galisim;

That, my ownership is being evidenced by a “Pagkakaloob Na Nagkakabisa sa Pagyao”

dated December 17, 1998 executed by the late Rosario L. Galisim in favor of the undersigned;

That on March 2, 2018, I executed an Affidavit of Lost alleging that said title was lost;

That the Affidavit of Loss was annotated at the back of the title under Entry No.
2018002820 dated March 02, 2018;

That, I found out that the subject title was transferred for safekeeping to Suzzette
Villaflor;

That upon knowing that the title is in possession of Suzzette Villaflor, I was able to
recover the said title from her;

That I am executing this affidavit to attest to the veracity of the facts contained herein
and for all legal intents and purposes it may serve.

I, SANTOS C. BAUTISTA, of legal age, Filipino, married, and with residence at 136
Bungahan, Malolos City, Bulacan, after having been duly sworn to in accordance with law
hereby voluntarily depose and say:

That on January , I executed an Affidavit of Loss involving the alleged loss of the
owner’s copy of Transfer Certificate of Title No. 108227 with an area of 487 square meters
registered in the name of the late Rosario Lopez Galisim;

That the Affidavit of Loss was annotated at the back of the title under Entry No.
2018002820 dated March 02, 2018;

That however, I recently recovered from one of my personal belongings the said title;

That I am requesting the Register of Deeds of Bulacan to annotate the Affidavit of

Recovery upon payment of legal fees;

That I am executing this affidavit to attest to the veracity of the facts contained herein and
for all legal intents and purposes it may serve.
