I am JERRAH LIM J. CORPUZ, RM 34 y/o a resident pf Brgy. Poblacion, Columbio, Sultan,

Kudarat. I am a graduate of Diploma of Midwifery at sultan Kudarat Educational Institution in the
year 2013 and Bachelor of Science in Midwifery Last 2019 in the same Institution.

As an employee of this Local Government Unit assigned in the MUNICIPAL HEALTH OFFICE
as Midwife assigned at COLUMBIO BIRTHING HOME and ANIMAL BITE TREAMENT
CENTER, I would like to signify my intent to continue my service in the Local Government Unit
of Columbio.

I believe that my experience for 8 years as a midwife in RHU has provided me with sufficient
experience and knowledge to perform the tasks and other additional functions entrusted to me.
Rest assured that I will do my best to perform duties and task with utmost professionalism and
dedication. Hereunder are my work experiences as an employee of this Local Government Unit.
