You are invited to take part in a research study entitled, “Relationship of Students’ Level of Confidence
and Academic Performance” conducted by Karl Ashley Carlos, Jo Lance Santarina, Ruth Joy Corpus,
Justine Lex Domingo, John Owen Domingo in fulfillment of their Undergraduate Requirement for
Bachelor of Science in Nursing. This survey is estimated to take an approximation of 5 to 10 mins to
complete.

The study aims to determine the Relationship of Students’ Level of Confidence and Academic
Performance in WU-P. In line with this, the study aims to determine if there is a significant correlation
between the Relationship of Students’ Level of Confidence and Academic Performance. The researchers
will also find solutions to help student nurses prepare better for their academic performance and level of
confidence.

This consent form explains the research study you are being asked to participate in. Before agreeing to
participate, please carefully read this form and ask about any concerns you may have regarding the
study. By completing this survey, you are confirming that you are 18 years 1-year-old and above.

The researchers uphold the protection of personal information given to us as a fundamental human right
of privacy. In accordance with Data Privacy Act of 2021, we are committed to promoting a safe and
secure space for our fellow students and executing appropriate actions in maintaining the confidentiality
and availability of your personal data. The researchers will only collect personal data that are willfully
