Subject: APPROVAL OF MEDICAL TREATMENT OF FAMILY

Dear Sir,
It is submitted that undersigned family is residing at Dunya Pur (Multan) along with my
parents. Being a low paid employee I cannot afford to relocate my family and parents in Islamabad. As
per reimbursement policy of outdoor medical treatment I am entitled to claim reimbursement of
amount Rs. 12000/- per financial year which is not sufficient in view of given inflation.

2. Therefore, it is requested that my family may be allowed to get medical treatment from
my native city and the medical expense incurred during the treatment may be reimbursed against the
bills provided from time to time.
