In view of our commitment to provide you with the best signal coverage and data services, DITO
will be requesting its facilities in Lanao del Norte.

 3 site in Sultan Naga Dimaporo (Karomatan)

 5 site in Lala
 2 site in Sapad
 1 site in Salvador

We would like to request to provide our facilities to support our equipment as designed.

Scope of works are as follows:

-Installation of New Transformer Solely dedicated to DITO including Drop Wire

-Installation of Service Entrance
-Wires from Service Entrance to ECB

In this regard, we hereby authorize our contractor, LEO Construction Inc., to apply, process,
secure or cause the issuance of the necessary permits, consent and other clearances for the said
power.
