October 19, 2022 for PICE National Convention which was attended face-to-face by the
undersigned last November 11 & 12, 2022 at SM City Bacolod, Bacolod City. The undersigned
humbly request for a reimbursement amounting to PhP 10, 575.00 as payment for the
registration fee, bus fare and per diem. Attached are the receipts and certificate of participation.
