I, LJ Kruger, the 16(2), appoint you Lezanne Marais, as the Manager responsible for the Health
and Safety of all Construction activities managed by the appointed Construction companies.

In terms of this appointment, you are responsible for Occupational Health and Safety matters at the
aforementioned areas. In order to ensure that you meet this responsibilities you must familiarise
yourself with the Occupational Health and Safety Act and its Regulations. You are also required
to ensure that all statutory requirements are met at all times and that the other Construction
appointees responsible for the construction activities comply with the Occupational Health and
Safety requirements and Construction Regulations applicable to your sites.
You may not further assign this duty.
This appointment is valid until you resign or the appointment is cancelled by me, LJ Kruger

You shall report directly to me LJ Kruger, on all occupational Health and Safety matters arising
out of your Projects construction activities.
