ACTIVITY DESCRIPTION: Hot work HDPE Membrane Installation Inside Tank Foundation. DATE OF ASSESSMENT:

TIME OF ASSESSMENT:
* Work area must be free of all Combustible materials removed from area.
* Fire extinguisher present at all times if in are where risk of fire exists and
trained person use the fire extinguisher.
* Ensure that all workers and operators have been properly trained in the
use of equipment and operational techniques
• Fire * Ensure that all Power tools, equipment and materials are properly

Pre-use inspection to ensure grinder, cables and electrical plugs are free
from damage and properly insulated;
* A cable management is to be applied to all powered electrical tools
(cables to be properly laid and fixed). At all times avoid the entangling and
trailing of cables;
* Equipment should not to be left running unattended;
* Guarding is provided and installed properly;
* Earmuffs, safety goggles and shockproof face shields provided and worn;
* Aprons to be used to restrict the sparks and other flying particles to get in
contact with body parts;
* Avoid loose clothing by properly buttoning of sleeves;
* Eyes and face wear protection resisting rating is to be considered to
withstand an impact from potential flying objects;
* Suitable, slip-resistant footwear to be worn;
* Cutting/Grinding wheels are to be changed by qualified person when the
equipment is switched off and unplugged from electrical source;
* Cutting/Grinding operations should be performed in designated area;
* Ensure the suitable discs are used for specific tasks and fixed as per
manufacture`s requirements;
• Existing cables * Use the correct type of disc, matching the axis`s speed of machine. Max
• Exposure of moving, rotating RPM of machine shall never exceed Max working RPM of the disc;

* Do not continuously use grinding wheel, allow grinding wheel and work
piece to cool down prior to handling;
* Grinding / Cutting disks shall be stored as per manufacture`s manual
guidelines;
* Ensure that damaged discs are properly segregated and disposed to
prevent any reusing;
* It is strictly prohibited to remove, modify, override or bypass by any
means the designed safety and protective devices of the equipment. Such
modified equipment is prohibited for use and must be marked as Non-
Operative and removed from the working place immediately;

erected where necessary;
