ACCOMPLISHMENT REPORT
ADOPT-A-SCHOOL PROGRAM

I. Introduction
A. Adopt-A-school to public school period. Refers to a program where a private entity agrees to
extend assistance in a particular aspect/s of its educational program within an agreed

Private Entity refers to a private individual or organization or business establishment whether a

corporation, a sole proprietorship, partnership or cooperative that enters to an agreement with a
public school in order to provide assistance in the implementation of the educational program. It
shall be known, henceforth, as adopting entity/agency.

B. Public Schools refers to a government school, whether elementary, pre-school, secondary, post-
secondary, technical/vocational institution. TESDA-registered training center or tertiary institution
that needs assistance and enters into an agreement with an adopting entity.

C. Assistance refers to the help given by the adopting entity to the public school. It may focus on
but may not be limited to staff and faculty development through formal education and training,
construction/upgrading of facilities, provision of books and other
instructional materials and equipment, and modernization of instructional technologies through
computers, television, internet, satellite programs, distance education, etc.
D. Agreement refers to the Memorandum of Agreement which specifies details of the adoption, i.e.
the time frame which shall be at least two years, assistance to be
extended, responsibilities of both contracting parties, etc.

II. Highlights of Accomplishment

A. Over all transformation of school facilities

The said activity was participated by different stake holders. A total of 187,000,00 cash donation
received from different donors and also a parcel of lot or 1,200 sq.m. or with market value of
1,200,000.00 donated in our school by Tuazon Family a resident of Brgy San Emiliano.
B. Portable handwashing donated per school by HPTA
C. School Entrance and exit gate was donated in our school by the Sangguniang
Barangay Official.
