SAMPLE SURVEY 1
As a parent or caregiver, your support for your child’s learning is important to your child’s success in
school and beyond. This survey asks for your opinions about what your child’s school does to partner
with you to support your child’s education.
Your responses will help your child's school improve their connections with families. All individual
responses will remain confidential. Results will only be reported as part of a group.
The survey usually takes no more than 10 minutes to complete. If you have more than one child in the
school district/building, please complete a survey for each child.
