JOINT AFFIDAVIT
(Re: Change of Venue of Marriage Solemnization)

We, JEROME and ANGELINE, both of legal ages and residents

of Barangay Rizal and/or Dumarais, after having been duly sworn to
in accordance with law, hereby jointly depose and state that:

1. We are the contracting parties who requested for the marriage

solemnization by Rev. Pastor Juanito, of the Word of Faith
Church;

2. We intend to be wed this January 22, 2023, Sunday, at nine

’clock in the morning;

3. We further intend to have our wedding be solemnized by Rev.

Juanito at Eva’s Eatery, Sto. Rosario, Zaragoza, Nueva Ecija on
the above stated date and time;

4. We are executing this Affidavit to attest to all the truth of the

facts aforestated for the purpose of compliance as regards the
change of venue for marriage solemnization as required
pursuant to Article 8 of the Family Code of the Philippines and
whatever legal purpose/s this foregoing document may serve.
