AFFIDAVIT

I, Mr. Santriwas Chaudhary, age 34 years, an Indian Inhabitant, resident of Krishna

nagar, Safed pool, Sakinaka, Mumbai-400 072, do hereby states and declare on the
solemn affirmation as under:-

1. I say that I am residing above said address along with family members.
 2. I say that my Son Shailesh Choudhary was born on 17th May ,2018 , at
AT/PO, Songadh Ledava, Dist. Siddharth nagar, Uttar Pradesh – 272 205
and her mother name Sushila Choudhary.
3. I say that my son (Shailesh Choudhary) who is applying for admission in
Yogiraj Shree Krishna Vidyalaya school in Primary section for academic
year 2023-2024.
4. I say that currently I do not have documentary proof of my son date of birth
and place of birth.
5. I am making this affidavit in order to produce the same before concerned
school authority to show my son Shailesh Choudhary was born on 17th May
2018 at AT/PO, Songadh Ledava, Dist. Siddharth nagar, Uttar
Pradesh – 272 205 and same is taken on record on the basis of this affidavit.

Whatever stated herein above is true and correct to the best of my knowledge and
belief and I believe the same to be true and correct and nothing is concealed by me.
