advised it was due to his son’s hospitalization. Trainer advised the
trainee to provide the medical certificate of his son to be considered
for possible re-wave.

PST Attendance 09/27/2022 – Absent. Trainee informed Trainer that he would resign
• 0 Absences
but did not submit resignation letter.

09/28/2022 onwards – NCNS. No resignation letter, medical

certificate nor further communication received from Trainee up to
date.

Overall, you have failed to meet the following performance metrics

• Product Training Attendance - Absences are not allowed during training. One (1) day of absence or
tardiness of forty five (45) minutes or more will result to disqualification in accordance with the stipulations of
your job contract.

Please arrange for the return of any Company property in your possession and process your clearance for the immediate release
of your final pay check and pro-rated 13th Month Pay, if any.
