AFFIDAVIT OF CAR MODIFICATION

I, PHILLIP JOSEPH CRUZ ONA, Filipino, of legal age, married, with

address at L3 B7, Millenium Village, Brgy. Bonbon, Butuan City, Philippines,
after having been sworn in accordance with law, hereby depose and state that:

1. I am the registered owner of a motor vehicle particularly described as:

 Make/Model: 2017 FORD ECOSPORT 5DR TR;

 Color : Frozen White;
 Engine No. : UEJAGY57294;
 Chassis No.: MPBSXXMXKSGY57294;
 Plate No. : ZAA1706.

2. Due to its limited cargo compartment capacity, there are personal items
and equipment that cannot fit inside the said motor vehicle;

3. There is a necessity to install a roof rack to provide a secure and

convenient way to transport items on the roof of my motor vehicle
particularly Aluminum Aerorack Roof Rack;

4. Aerorack Roof Rack is recognized as one of the recognized after-market

brands of roof basket to be safe, and will not compromise the integrity and
safety of the vehicle;

5. The roof rack shall not be loaded more than its recommended load
capacity; and

6. I am executing this affidavit for purposes of attesting to the truth of the

foregoing and to support the registration of my motor vehicle including
the above modification.
