SPECIAL POWER OF ATTORNEY

Filipino, and a resident of Block 7 Lot 1 Iligan Bay Vista Village Paitan
Dalipuga Iligan City, by virtue of these presents, do hereby name,
constitute, and appoint CATHY QUISIMUNDO CARDOZA, single, of
legal age, Filipino, with residence and postal address at Block 7 Lot 1
Iligan Bay vista Village dalipuga, Iligan City, to be my true and lawful
Attorney-in-Fact, for me and in my name, place, and stead, to do and
perform the following acts and things, to wit:

1. To represent me in the processing of my change of name for

my PSA Birth Certificate, Voter’s Affidavit/Certificate,
Affidavit of unemployment and all other requirements
directly or indirectly associated with the processing and do
other necessary acts that may be required by the Office of
the City Civil Registrar in the compliance or requirements as
prescribed under RA 9048;

2. To make, execute, sign documents and/or instruments

relative to the aforesaid authority.

HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full

power and authority to do and perform any and all acts necessary or
requisite or proper to be done in and about the premises, as fully to all
intents and purposes as I might or could lawfully do if personally present,
and HEREBY RATIFYING AND CONFIRMING all that my said Attorney-in-
Fact shall lawfully do or cause to be done under and by virtue of these
presents.
