Poland Criminal Record Process

Introduction
Criminal Record searches in Poland are conducted through a designated agent who submits the request
directly to the National Authority MINISTERSTWO SPRAWIEDLIWOŚCI, BIURO INFORMACYJNE, KRAJOWEGO
REJESTRU KARNEGO (Ministry of Justice National Crime Register (NCR)), requesting that such a Certificate be
issued. This packet provides the required forms and instructions to complete this process.

Application Procedure for Subjects

The subject is required to fill out 2 forms:
 Specific Application form in Polish (ZAPYTANIE O UDZIELENIE INFORMACJI O OSOBIE) - This is the official
NCR request form.
 Bilingual Authorization To Authorise our agent to submit the application on the subject’s behalf. Please
note that if the subject is Hungarian then a copy of the Authorisation Letter is required in Polish and
Hungarian.
 Current Address
*A copy of the ID is not a mandatory requirement but recommended to ensure all information entered in the
application is correct. If the name is not clear in the application form there is a change of rejection by the
Authority.
Results will be returned directly to our partner, who will enter them into our system for delivery to the
customer.

Additional Fees
There are no additional fees for this search.

Turnaround Time
The turnaround time averages 8 business days but additional time may be required for foreigners. This is
because the Authority reach out to the country of birth as part of the application process.

ZAPYTANIE O UDZIELENIE INFORMACJI O OSOBIE

This form is the official request to the NCR to release your criminal record history.
The Polish form should be filled out and submitted with the search request. An English translation is provided
for the convenience of our English speaking customers and subjects. This English form should NOT be filled
out.
The data subject must fill out the form completely. This form should not be filled out by the employer or
organization.

 PESEL: This is national identification number used in Poland. Most people who have lived and
worked in Poland will have this number and is required for this form. If you do not have this number,
please provide passport number

 Questions 1 – 10: Please enter your information in these questions. For question 6, Date of Birth,
please make sure to enter the date in the European date format of dd/mm/yyyy.
 Question 11 – Leave this question blank. This is for the internal use of the NCR.
 Question 12 – Please check box #1 for Karoteka Karna (Crime record).
 Question 13 – Leave this question blank. This is for the internal use of the NCR.
 Be sure to sign the form when it is completed.

When entering your Date of Birth, be sure to use European Date Format of dd/mm/yyyy.
