NOTIFICATION TO REMIT FEDERAL FUNDS

Dear «Payee Name»,

The North Carolina Office of Recovery and Resiliency (NCORR) is North Carolina’s administrating agency for the
Housing Opportunity and Prevention of Eviction (HOPE) Program funds. The HOPE Program consists of U.S.
Federal funds to grant emergency rent and utility assistance in response to the COVID-19 pandemic. The
Program is required to recapture funds that are not properly issued, obtained fraudulently, or when there is a
Program or Landlord Tenant Agreement violation.

NCORR has determined, through third party verification, the documentation submitted to support eligibility for
HOPE Program assistance was faulty. Therefore, NCORR has found the application to be ineligible and requires
the awarded funds be returned or repaid.

As the payee of record, NCORR hereby formally notifies you of the Program’s intention to recover funds
awarded in the amount of $_______ directly from you or your organization.

You have thirty (30) calendar days from the date of this notice to refund the payment, in the amount above to the
following address:

If you have verifiable documentation to refute this claim, contact the Program immediately, and no later than thirty
(30) calendar days, by email to RepaymentsNCORR@ncdps.gov.

If you wish to exercise the option to enter into a repayment agreement immediately, please sign the attached
HOPE repayment agreement (see Exhibit 1).

Failure to respond to this letter and refund the federal funds requested could result in NCORR taking legal action
or referring this matter for debt collection.
