Findings:

- Evidence of heterogeneous pleural-based (involving all the
pleural parts; including coastal, mediastinal, and diaphragmatic
pleura) lobulated soft tissue mass lesion seen encasing and
compressing most of the left lower lung lobe with some extension
to the major fissure, the largest part measuring roughly 17 x 9.3
x 16 cm which appears non-separable from the pericardium
associated with mild pericardial effusion. The medial border of
some of the surrounding ribs appear eroded by the mass.
Correlation with soft tissue biopsy is advised.
- Emphysematous changes seen mainly in both lung apicies.
- Multiple enlarged prevascular, paratracheal, pretracheal,
paraaortic, left cardiophrenic, and left hilar lymph nodes, the
largest is left paratracheal measuring about 1.7 cm in its short
axis.
- Evidence of central line with its tip seen in the right atrium with
few foci of subcutaneous gas seen in the left supraclavicular
region.
- Cardiac size is within normal limits with diseased coronaries
noted.

- The visualized portion of the skeleton shows severe


degenerative changes of the examined spine including multi-level
anterior bridging osteophytes.
- Severe atherosclerotic changes of the thoracic aorta and its
major branches.
- The visualized portion of the upper abdominal organs shows:
- Multiple calcified granulomas seen in the liver.
- Multiple gallbladder stones. The largest measuring about 3.5 x
2.2 cm.
- Multiple bilateral renal cysts, The largest is in the upper pole of
the right kidney measuring about 2.3 x 2.1 cm. Some of them
have hyperdense components suggestive of hemorrhagic cysts.
- Multiple bilateral retrocrural, left cardiophrenic, gastrohepatic,
peri and para-aortic and porta hepatic enlarged lymph nodes.
