MEDICAL REQUIREMENTS AND PROCEDURES

FOR AUSTRALIAN VISA APPLICANTS

LIST OF REQUIREMENTS
1 Health Examinations List (HAP letter with HAP ID) or Referral Letter (with TRN) from the Australian Embassy
2 Current Valid passport and photocopy of the Bio Page
*If passport is not available, please consult with us before your appointment date via email at mhc.au@iom.int.
3 One (1) Government-issued ID for the building security at the Ground Floor

ADDITIONAL REQUIREMENTS
For Applicant 0-18 yrs. old: For applicants who have been vaccinated:
